Job summary

LaSante Health Center is seeking an Optometrist to provide comprehensive eye care services in a federally qualified health center (FQHC) setting on a per diem basis. The position focuses on high-quality vision care while collaborating with primary care and specialty teams to support overall patient health.

Responsibilities include performing comprehensive eye examinations, diagnosing and managing common eye conditions, prescribing lenses, and educating patients on eye health.

Qualifications

  • OD degree from an accredited program.
  • Current New York State optometry license (or eligibility).
  • Experience in outpatient or community health settings preferred.
  • Strong communication and patient engagement skills.
