Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Active OT license required.
• Experience with seniors in skilled nursing/post-acute settings is preferred.
• Ability to develop and implement individualized treatment plans.
• Documentation in EMR and collaboration with an interdisciplinary team essential.
