Job summary

Community Physicians in Jefferson, Wisconsin is seeking an onsite Patient Care Navigator to serve as a vital bridge between clinicians, staff, patients and families, ensuring seamless transitions of care across settings.

This full-time role offers 8-hour shifts, Monday through Friday, with pay from $21.50 to $30 per hour, plus a benefits package including health/dental/vision, HSA, life insurance, generous PTO and travel reimbursement.

Qualifications

• High school diploma or equivalent is required.
• Graduate of an accredited Medical Assistant program is required.
• CMA certification is highly preferred.

Responsibilities

• Assist with patient transitions to the next level of care and schedule follow-ups.
• Perform chart creation and data entry of demographics, medications, and diagnoses in the EMR.
• Support transition clinic services, patient intake, calls, and documentation.
• Attend interdisciplinary team plan of care meetings and communicate with providers.
