Qualifications

• Associate’s degree in nursing or higher required
• Current Iowa nursing license by date of hire
• Excellent written and verbal communication skills
• Proficiency in Microsoft Excel and clinical documentation

Responsibilities

• Assess patient nursing care needs and evaluate outcomes
• Coordinate continuum of care across settings
• Develop and implement patient care plans based on condition and family considerations
• Document interventions and patient responses
• Collaborate with multidisciplinary team to ensure quality care
• Assist with discharge planning and care transitions
• Follow department policies to maintain patient safety and care quality
