Intermountain Health’s Primary Children’s Hospital in Salt Lake City, Utah, seeks a social worker to address medical and clinical social work needs, perform psychosocial assessments, and participate in the interdisciplinary care plan.

Utilizes social work expertise to address medical and clinical social work needs for a defined patient population. Performs psychosocial assessments and contributes to the interdisciplinary plan of care by developing and implementing a plan of care/treatment plan that provides extraordinary care for the patient while being sensitive to costs and resources. Works collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties. May also be accountable for the management of discharge/transition planning needs in some settings.

Hourly range for this position: $35.60 - $54.93 per hour, based on experience. Scheduled weekly hours: 40. Full-time, Monday through Friday, Day shift.
