Qualifications

• Nursing Diploma or Associate's degree required.
• RN licensure/certification required or eligible.
• BSN or ADN welcome to apply.

Responsibilities

• Provides direct patient care including assessment, planning, implementation and evaluation.
• Educates patients and families on illness, treatment, home care and medications.
• Promotes patient safety and quality through compliant practices.
• Collaborates with interprofessional team to develop and adjust care plans.
• Maintains communication of the plan of care across the continuum.
