The role requires a nursing program graduate, current RN license, and the ability to work flexible hours including days/evenings/nights.

Qualifications

• Graduate of a nursing program recognized by ACEN/CCNE/NLN CNEA.
• Current RN license by applicable state requirements.
• One year of RN experience required.
• Ability to work flexible hours including days, evenings, nights, holidays, weekends.

Responsibilities

• Coordinate nursing care across the continuum for assigned patients.
• Provide patient/family education and discharge planning.
• Support transitions of care and care coordination activities.
• Demonstrate leadership and collaboration within the nursing team.
