You will assess member needs, develop and implement care plans,coordinate services, and monitor progress while containing health care costs. Requirements include 5 years of related experience and RN licensure or multi-state license; preferred are BSN and specialty experience.

Qualifications

• 5 years in any combination of clinical, case/utilization management, and/or disease condition management experience.
• RN licensure required or eligible for multi-state licensure via eNLC.

Responsibilities

• Manage a caseload of members with complex health needs, requiring advanced clinical knowledge.
• Develop individualized care plans in collaboration with members and providers.
