Job summary

The ideal candidate will hold a California Pharmacist License, AHA BLS certification, and have residency or BCOP credentials.

Qualifications

• CA Pharmacist license required.
• AHA BLS required within 90 days of hire.
• Oncology/hematology residency or equivalent preferred.

Responsibilities

• Provide comprehensive pharmaceutical care in oncology/hematology for outpatient infusion patients.
• Perform formal clinical pharmacotherapy consults with documentation of actions and outcomes.
• Participate in oncology/hematology committees and cross‑functional teams to improve patient care.
