Job summary

The Nurse Practitioner at the Greenville Health Care Community Based Outpatient Clinic provides comprehensive cardiology patient care across invasive, non-invasive, consultative, and inpatient/outpatient settings. The NP reports to the Chief of Oncology Department to ensure high-quality medical service delivery.

Qualifications

• Must be a licensed independent practitioner with advanced education and national certification.
• Experience in cardiology care, both inpatient and outpatient, with emphasis on health promotion.
• Ability to lead within an interdisciplinary team and coordinate with specialty services.

Responsibilities

• Independently manages a patient panel with acute and chronic conditions focusing on health promotion and disease prevention.
• Develops and implements individualized care plans for population health and disease management.
• Orders and interprets labs and diagnostic tests; prescribes pharmacologic and non-pharmacologic interventions.
• Initiates referrals and consultations to specialty services as needed.
• Provides education to patients and families and adjusts care plans to optimize outcomes.
