Job summary

Texas Oncology/Texas Breast Specialists in Austin Midtown is seeking a Nurse Navigator (RN) to join our team. This full-time, Monday–Friday position supports patients across the cancer care continuum, providing education, access to resources, and care coordination within a community oncology setting.

You will work with physicians, nurses, and staff to identify gaps in care, lead care conferences, monitor risk, and guide patients through treatment and survivorship.

Qualifications

• Bachelor’s prepared RN (BSN).
• Oncology Certification (OCN) preferred.
• Nurse Navigator Certification a plus.
• 5 years RN experience in oncology.
• Demonstrated leadership and collaboration skills.
• Ability to apply the nursing process in clinical operations.
• Ability to assess health disparities and address community needs.
• Strong organizational and communication skills.
• Experience with care coordination and multidisciplinary teams.
• Proficiency with electronic medical records.

Responsibilities

• Coordinate care across the cancer care continuum.
• Identify gaps in care delivery and seek closures.
• Lead Patient Care Conference (huddle) and communicate outcomes.
• Assess patient risk and perform psychosocial assessments.
• Provide chemotherapy education and monitor adherence/side effects.
• Support survivorship and advance care planning visits.
• Contribute to research, quality, and performance improvement.
• Engage in patient advocacy and community outreach.
• Familiarity with practice operations and healthcare economics.
