Job summary

Community Health Network in Indianapolis, IN is seeking an Oncology Nurse Navigator to join the North Cancer Center and coordinate patient care across the oncology continuum. You will educate patients about disease processes and monitor care to ensure timely, coordinated services.

This role requires RN licensure in Indiana, two years of oncology nursing experience, and certification progress (OCN preferred; ONN-CG within the timeframe set at hire).

Qualifications

  • Two years or more of Oncology nursing experience required.
  • OCN certification preferred but not required upon hiring; ONN-CG certification required within timeframe established upon hire.
  • Licensed as a Registered Nurse (RN) with Indiana licensure; BSN preferred.

Responsibilities

  • Work as part of a multi-disciplinary team to identify patient needs and coordinate services across the care continuum.
  • Educate patients on disease processes and monitor/coordinate care for timely care delivery.
  • Facilitate access to available resources to ensure seamless navigation through the oncology journey; performs duties at the North Cancer Center.
