Inova Health is seeking an RN Oncology Navigator for the Riverstone Clinic in Breast Surgery, a PRN day shift role (Mon–Fri, 8:00am–5:00pm) in Fairfax, VA. The position supports community cancer education, care coordination, and patient advocacy within a leading nonprofit health system.

Minimum qualifications include a BSN, Virginia RN license, 3 years’ primary care nursing experience, and BLS (OCN within 2 years).

Qualifications

• Bachelor's degree in Nursing is required.
• Licensed in Virginia as a Registered Nurse.
• Minimum 3 years progressive nursing experience with primary care management.
• BLS certification; OCN within 2 years of employment.

Responsibilities

• Promote healthcare programs and cancer education to community resources.
• Assess patient health and psychosocial needs; develop a care management plan with patient and support system.
• Coordinate patient's plan of care and aiding with referrals, scheduling, and discharge planning.
• Document pretesting plans, orders, consent, and outcomes per policy; communicate with team.
• Collect data to support regulatory compliance and contribute to outcome measures and patient satisfaction surveys.

Inova Schar Clinic is looking for a dedicated RN Oncology Navigator to join the team in their Riverstone Clinic for Breast Surgery. RN Oncology Navigator Job Responsibilities

• Conducts ongoing promotion of healthcare program to community-based resources to include developing written materials and public presentations describing activities related to patient care and research. Coordinates community health programs and cancer-related education and groups.
• Conducts comprehensive assessment of the patient's health and psycho-social needs, and develops a case management plan in collaboration with the patient and their support system. Assesses patient needs and coordination of multidisciplinary care. Serves as a clinical consultant to the health care team.
• Coordinates a patient's plan of care which may include answering patient questions regarding disease process and plan of care, appointment scheduling when appropriate, assisting with referral process, and providing community resources. Coordinates patient discharge from hospital to insure adherence to SOPs. Maintains a working knowledge of availability of community resources and financial services for providing clinical care and knowledge about appropriate referrals. Educates patients and family members regarding disease process.
• Completes documentation related to pretesting, plan, written consent, orders and outcome according to policy; communicate verbally or in writing to other team members.
• Organizes and submits data to ensure compliance with regulatory guidelines. Incorporates data and information to continuously improve services and practice to enhance outcomes. Participates in the development of outcomes measures as well as surveys regarding patient satisfaction with the services received, services provided, and the impact of patient education.

Minimum Qualifications

• Education: Bachelors Degree in Nursing
• License: Licensed in the Commonwealth of Virginia as a Registered Nurse
• Experience: 3 years progressive nursing experience with primary care management of patient population
• Certification: BLS from the American Heart Association. OCN within 2 years of employment

Preferred Qualifications

• Previous oncology experience strongly preferred
