Location: VMC Main Campus, Renton, WA

Shift: Days (Hours: 7am-5:30pm)

Type: Full Time (FTE: 1)

Salary Range: Min $94,894 - Max $142,341/annual DOE

Job Overview

Guides patients through the health care system by assisting with access issues, developing relationships with service providers and tracking interventions and outcomes. Promotes a positive teamwork environment that emphasizes respect toward all customers, including patients, families, physicians, staff, and other departments. Receives and responds to calls from patients and families for health care assistance using the nursing process in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization. This position also works collaboratively with clinical care teams to coordinate care for certain patient symptoms and conditions.

Prerequisites

• Graduated from an accredited nursing program. Bachelor of Nursing degree preferred.
• Current Washington Registered Nurse (RN) License.
• Minimum two (2) years nursing experience in Oncology or five (5) years nursing experience required.
• Preferred: Nursing experience in outpatient clinic setting.
• ONCC certification preferred.

Essential Responsibilities and Competencies

• Facilitates patients’ entry into the diagnostic and/or treatment system and coordinates their care, including family involvement.
• Follows patients throughout the continuum of health care services across multiple departments, acting within scope of practice.
• Implements risk assessments and healthcare pathways for cancer patients based upon clinical staging.
• Evaluates progress of patients and is responsible for professional and personal modifications as directed by the health care team.
• Develops relationships with all oncology providers and serves as liaison between patient, hospital departments, team members, and providers.
• Comprehensively assesses the patient’s physical and psychological status.
• Provides approved patient education and symptom management, and delivers test results as directed by physicians.
• Provides access to community resources and links patients to support services such as social services, nutrition services, psychologist, pastoral care, physical therapy, rehabilitation therapy, financial counseling, etc.
• Assists with data management and entry for tracking quality metrics for diagnosing and treating cancers promptly and effectively.
• Develops and implements approved workflow processes for the center to deliver high‑quality care to patients within the community.
• Participates in the survivorship program.
• Coordinates site‑specific cancer patient multidisciplinary visits, documents treatment plan summary, and disseminates to patient, departments, and physicians.
• Participates in site‑specific cancer multidisciplinary tumor conferences; documents recommendations and monitors for appropriateness with NCCN or other national guidelines.
• Participates in the national accreditation process for the cancer center program.
• Develops, in conjunction with support staff, cancer support groups and education programs for patients, their families, or caregivers.
• Participates in the Patient Advisory Committee and /or Patient and Family Centered Care Committee.
• Participates in process improvement work that supports quality and safety initiatives.
• Functions as patient advocate as initial contact for patient‑related issues.
• Provides support and coverage for clinic care management team.
• Additional duties as assigned.

Telephone Assessment, Triage, and Care Management

• Manages patient health concerns and symptoms via telephone interaction utilizing approved protocols for assessment and management.
• Performs in‑patient assessment, care coordination and professional nursing functions to assess severity of patient health concerns and provide disposition to emergency care or clinic provider.
• Gathers objective and subjective data from the patient.
• Executes medical regimens and nursing interventions as appropriate for the setting and competency.
• Contacts emergency response services when appropriate.
• Provides referrals to healthcare providers and community resources, including locations, hours, and contact information.
• Schedules appointments as needed.
• Provides education for patient and/or family members according to individualized needs and policy.
• Responds to patient questions and provides reassurance.

Documentation in the Electronic Health Record

• Provides and documents accurate and detailed assessment in patient records.
• Enters all pertinent communications with the patient into the Medical Record.
• Navigates and documents in the electronic health record while simultaneously talking with patient or family member on the phone.
