Advocate Lutheran General Hospital in Illinois is seeking a Medical Social Worker (Home Health) for a part-time schedule (20 hours per week). We’re Looking For: Licensure or Certifications License/Registration/Certification: IL locations-LSW Eligible, LCSW preferred issued by the State of Illinois WI locations -Social Work Certification issued by the State of Wisconsin Education/Experience: Masters of Social Work services from an accredited school Years of Experience: 1-2 years of experience in a clinical setting post licensure preferred. Basic proficiency with computers, including Microsoft Word and email Ability to learn and effectively use electronic scheduling and documentation systems Strong written and verbal communication skills with attention to professional documentation Excellent interpersonal skills Effective time management and organizational abilities A valid driver’s license issued by the Division of Motor Vehicles

Extensive travel within geographical boundaries in all extremes of weather and on all types of roads including expressways. Must be able to provide/arrange own transportation to and from patients’ homes and branch office. Ability to work rotating schedules as needed. Requires adherence to scheduled days and hours of work based on commitment to patient/client and workload assigned. If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
