Job summary

Responsibilities

• Performs initial and ongoing assessment of the patient, utilizing appropriate data.
• Demonstrates clinical judgment in responding to priority patient needs.
• Communicates and documents assessment of patient with the interdisciplinary team.
• Develops and documents plan of care, treatment and services which is individualized and appropriate for patient needs.
• Provides care in collaboration with interdisciplinary health team and educates patient and family.
• Evaluates patient responses to care and updates documentation accordingly.
• Ensures patient and family education using teach-back method and documents appropriately.
• Leads care delivery team with effective communication and collaboration.

Education

Diploma or Associate’s Degree in Nursing

Bachelor’s Degree in Nursing preferred
