Oncology Infusion Pharmacist – Banner Health, Phoenix, AZ

Position Summary

This full‑time, day‑shift position (8:00am‑4:30pm, five 8‑hour shifts per week) is based at the Banner University Medical Center – Phoenix MD Anderson campus. The pharmacist will deliver pharmaceutical care to oncology patients within a multidisciplinary team and oversee the operations of the infusion pharmacy. This includes managing hazardous IV compounding, coordinating workflow, and mentoring pharmacy technicians, interns, and students.

Core Functions

• Process medication orders and evaluate their appropriateness and completeness relative to patient data (age, height, weight, diagnosis, laboratory values, etc.).
• Use clinical pharmacy skills to assess patient medication regimens and formulate recommendations that ensure optimal safety, efficacy, and cost‑effectiveness; communicate with prescribers as needed and resolve medication order problems.
• Serve as a resource for patients and the health‑care team, providing timely medication information.
• Comply with directives of the Pharmacy and Therapeutics Committee/Clinical Consensus Group and document all patient‑care activities in the electronic medical record and other relevant databases.
• Prepare and dispense medications, including sterile or non‑sterile compounded products, in an accurate and timely manner in compliance with company policies, procedures, and regulatory requirements.
• Oversee daily pharmacy operations, manage technology and inventory, and supervise staff.

Minimum Qualifications

• Unrestricted pharmacist licensure in the state of practice and a degree in Pharmacy from an accredited College of Pharmacy.
• For Arizona candidates: must provide an Arizona Board of Pharmacy Wallet Card at time of hire and be licensed within 30 days of the job offer, with licensure completed within 90 days unless delayed by the State Board.
• Experience: at least one year in an acute‑care hospital setting, with the last 12 months of practice in an acute‑care specialty area.
• Strong organizational, problem‑solving, computer, written and verbal communication skills; proficiency with medication management software, Microsoft applications, and automated dispensing systems.

Preferred Qualifications

• Completion of an accredited pharmacy residency program or prior clinical pharmacy experience.
• Prior hospital experience.
• Additional related education or experience.
