Job summary

You will provide patient-centered care and integrated pharmacy services in a decentralized practice with physicians, nurses and hospital staff. This role involves supporting medication-use systems and delivering safe, effective therapy across assigned areas, with requirements including ACPE-accredited education, PGY residencies or equivalent experience, BPS certification, and an active state

Qualifications

• Doctor of Pharmacy or B.S./PharmD from ACPE-accredited school.
• Completion of PGY-1 residency; PGY-2 in specialty or 3+ years clinical experience.
• BPS certification or within 1 year; active license in good standing.
• State requirements such as sterile compounding; BLS/ALS/ACLS credentials.

Responsibilities

• Provide safe, effective, and prompt medication therapy.
• Deliver patient-centered care and integrated pharmacy services with clinicians.
• Participate in medication-use system in decentralized and centralized settings.
• Support overall medication-use systems for the department.

Clinical pharmacy experience

Hospital pharmacy

Education

B.S. or Pharm D. from ACPE-accredited School of Pharmacy

PGY-1 Pharmacy Residency or fellowship

PGY-2 Specialty Pharmacy Residency or fellowship

3+ years clinical pharmacy experience (if no PGY-2)

5+ years clinical pharmacy experience (no residency/fellowship)

Board of Pharmacy Specialties (BPS) certification (or within 1 year)

Active Pharmacist license in good standing

Sterile compounding / other state requirements

BLS/ALS/ACLS certifications
