Job summary

UPMC Community Osteopathic Pharmacy is seeking a Clinical Pharmacist for Oncology to deliver safe, effective medication management across centralized and decentralized services, collaborating with interdisciplinary care teams.

The role includes patient care, optimization of therapy, education, and participation in research, quality initiatives, and precepting students and residents, with sign-on bonus up to $10,000.

Responsibilities

• Leads assigned patient service lines, clinical areas, and therapeutic programs.
• Facilitates pharmaceutical care services and medication utilization systems.
• Designs and implements stewardship activities and surveillance programs.
• Tracks and evaluates programs for quality, safety, and cost.
• Provides discharge medication review, reconciliation, and counseling.
• Maintains proficiency in hospital computer systems and medication ordering.
