This role supports the multidisciplinary Supportive Oncology team and emphasizes patient-centered communication and HIPAA compliance.

Qualifications

• High School diploma or GED and 3+ years in healthcare or ambulatory setting.
• 2 years as navigator or navigator + care coordinator with knowledge of program coverage.
• 2 years of EMR experience.
• Proficiency with Microsoft Office programs required.
• Advocacy and cultural mediation/competency.
• Strong written, verbal and interpersonal communication for patients and families.
• Ability to multi-task, work independently and stay organized.

Responsibilities

• Develops action plans and basic care planning for low-risk patients.
• Monitors inpatient/discharge lists and assists with transportation needs and barriers to appointments.
• Conducts patient outreach to ensure timely care delivery.
• Assists with insurance needs and connects patients with resources for enrollment.
• Improves and develops workflows within the team.
• Educates patients on available resources and SDOH programs.
• Ensures compliance with quality measures and HIPAA regulations.
• Manages patient caseloads and collaborates with multidisciplinary team.
• Provides training for navigators and shares reference materials.
