Job summary

Frederick Health Hospital, Inc. is seeking a full-time Care Manager Social Worker for our Oncology Services in Frederick, MD. The role supports care coordination across inpatient and ambulatory settings, with hours typically 8:00 AM to 4:40 PM or 8:30 AM to 5:00 PM, Monday through Friday.

The ideal candidate holds a Maryland social work license (MSW preferred) and has 3–5 years of care management experience. CPR is required; ACM certification is a plus.

Qualifications

• Graduation from an accredited school of social work with a Bachelors or Master’s degree in social work and a current Maryland license.
• Master’s Degree preferred.
• Care Management certification preferred (ACM etc.).
• CPR certification required (not for remote role).
• Completes Maryland Board of Social Work CEU requirements.

Responsibilities

• Coordinate care and assist with navigation across the healthcare continuum.
• Perform psychosocial assessments and develop care plans to address identified needs.
• Communicate with interdisciplinary care teams, patients, and families.
• Monitor and evaluate care management plans and outcomes.
• Assist with discharge planning and transitions of care.
