Job summary

UNC Health Care's Hematology Oncology Clinic Cary is seeking a flexible RN Care Manager to coordinate care for complex oncology patients. The role combines assessments, care planning, transitions, and payer communication in a hybrid, full-time setting.

You will collaborate with clinicians, patients, and families, using EHR tools to ensure timely interventions, documentation, and high-quality patient outcomes in a 40-hour work week.

Qualifications

• Graduation from a state-accredited school of professional nursing.
• Registered Nurse licensed to practice in North Carolina.
• Minimum 2 years RN experience in oncology.

Responsibilities

• Conduct comprehensive patient assessments and identify barriers to care.
• Collaborate with providers and families to develop individualized care plans.
• Utilize standardized tools for risk segmentation to guide interventions.
• Coordinate tasks and interventions across the care team and community resources.
• Facilitate care transitions and multidisciplinary team meetings.
• Communicate with payers to support timely approvals and appropriate care.
• Accurately document services and support billing as required.
• Provide longitudinal care within the Medical Home framework.
