Job summary

Infinite Medical P.C. seeks a full-time, on-site Licensed Nurse (RN) to serve as a Nursing Care Coordinator in Vinings, GA. You will review chart documentation to ensure CMS compliance and support seamless continuity of care for residents.

The role emphasizes care management, documentation accuracy, and collaboration with HR, Operations, and Clinical teams, using PCC, MatrixCare, and other EMR systems.

Qualifications

• RN license in good standing
• Experience with geriatric care and chronic conditions
• Strong knowledge of medical terminology and documentation
• Familiarity with CMS guidelines and RVLs

Responsibilities

• Review patient records and RVLs for CMS compliance
• Identify documentation discrepancies and propose corrections
• Coordinate with HR, Operations, and Clinical teams
• Maintain proficiency with primary digital platforms (PCC, MatrixCare, SigmaCare, Google Workspace)
