Qualifications

• Licensed physical therapist with active state license.
• Experience delivering PT plans and documenting progress in EHR.
• Ability to perform initial assessments and establish care plans.
• Adherence to safety, compliance and professional standards.
• Strong time-management and on-site presence.
• Proven ability to work in a high-visibility, target-driven environment.
• Collaborate with multidisciplinary team for coordinated patient care.
• Educate patients and families on home exercise and injury prevention.
• Identify territory opportunities for service expansion and growth.
• Report outcomes to supervisors for strategic decisions.

Responsibilities
