Job summary

Qualifications

Responsibilities

• Provide high-quality NP clinical care on-site, including assessments and treatment plans.
• Develop and manage patient pipelines to ensure steady flow and clinic targets.
• Document all patient interactions accurately in EMR.
• Meet or exceed performance targets through proactive patient engagement.
• Coordinate care with physicians and team members.
• Lead patient education and promote self-management.
• Utilize data analytics to identify care gaps and drive targeted interventions.
