Job summary

Pathways Healthcare is seeking an On Call Hospice Registered Nurse to join our in-home care team in Kingston, PA. This part-time role covers weekend and day shifts, delivering compassionate nursing care in patients’ homes.

Responsibilities include administering meds, revising plans of care as needed, and coordinating with physicians and the care team to ensure quality outcomes. A geriatric focus and solid EMR skills are preferred.

Qualifications

• 2+ years' nursing experience, preferably in geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable vehicle.

Responsibilities

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assesses patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.
