Shift 5:00 PM – 8:00 AM

The Registered Nurse (RN) will be responsible for providing care and services for a group of hospice patients, visiting patients where they reside, which could include private homes, assisted living facilities and hospitals. The RN will complete hospice admissions and patient visits as needed, answer calls from patients, families and customers, make nursing visits for symptom management, deaths, and admissions as needed, and advocate for hospice services and Suncrest Hospice. The position is after‑hours on‑call working seven days/nights on and seven nights/days off.

Qualifications:

  • Current RN licensure (in good standing) in the state(s) of practice with at least one year of experience under license
  • Admissions and on‑call experience preferred
  • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
  • Work closely with patients on a 1‑to‑1 level while receiving strong support from a patient care team
  • Can work independently and manage own activities including phone, email, written communication, and meetings with a variety of healthcare professionals
  • Time‑management skills to manage visits, documentation, meetings, and urgent needs by prioritizing and triaging multiple concerns
  • Ability to learn an electronic medical records system
  • Willingness to drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area
