Job summary

Commonheart is seeking a registered nurse (RN) licensed in Wisconsin to join our Home Health and Hospice team. The role involves in-home patient care, with a travel requirement and a company car provided.

Qualifications

• RN license in Wisconsin is required.
• Current, valid driver’s license is required.
• Minimum of 1 year experience in home hospice or home health preferred.

Responsibilities

• Provide in-home hospice or home health care to patients.
• Coordinate care with interdisciplinary team to meet patient needs.

Job Status

• Full Time

Salary

• $75K to $80K, based on experience

Schedule

• 7 on / 7 off, Monday-Sunday
• Monday - Friday: 4PM - 8AM
• Weekend: Saturday at 8AM to Monday at 8AM
• A combination of day and evening shifts

Location:

Hit the road without the extra costs—this role includes travel and a company car is provided.
