Job summary

Luminary Hospice in Texas seeks an after-hours registered nurse to support patients and families in their homes during evenings and overnight hours, with a rotating schedule: one week on, one week off. This role delivers comprehensive nursing care, pain management, and emotional support as part of an interdisciplinary team.

You will assess patients, develop care plans, administer medications, educate families, and coordinate with Case Managers to ensure quality end-of-life care.

Qualifications

  • Bachelor's or Associate's degree in nursing or related fields, required.
  • Active nursing license in good standing in the state performing job duties.
  • Active CPR certification.
  • 2-5 years of experience in healthcare management, hospice, or palliative care, preferred.
  • At least 1 year Medicare Hospice Certification and CHAP guidelines knowledge, preferred.
  • Knowledge of hospice care services provided to patients and families.

Responsibilities

  • Completes initial and comprehensive assessment of patient and family to determine hospice needs.
  • Provides nursing care using all elements of the nursing process and health assessment data.
  • Assesses and evaluates patient status and revises the plan of care as needed.
  • Develops a care plan with goals based on nursing diagnosis and family involvement.
  • Administers medications and treatments as prescribed by the physician.
  • Assists patient with activities of daily living and supports self-sufficiency and comfort care.
  • Coordinates patient care with Case Managers after hours.
  • Maintains and submits accurate clinical notes and tracks pain/symptom changes.
  • Communicates with physician about patient needs and orders.
  • Teaches patient and family self-care techniques as appropriate.
  • Supports family/caregiver through interdisciplinary team involvement.
  • Participates in quality improvement activities.
  • Performs additional duties as assigned.

About the role:

The after-hours registered nurse supports the patient needs of the agency in the evening during the weekdays, working overnight shifts Monday through Thursday (5:00 PM - 8:30 AM) and a continuous weekend shift from Friday at 5:00 PM through Monday at 8:30 AM. This role follows a rotating schedule: one week on, followed by one full week off. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities after normal business hours.

Job Responsibilities:
  1. Completes an initial and comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
  2. Provides professional nursing care by utilizing all elements of nursing process and utilizes health assessment data to determine nursing diagnosis.
  3. Assesses and evaluates patient's status through writing and initiating plan of care, re-evaluating patient and family/caregiver needs after hours when needed, and participates in revising the plan of care as necessary.
  4. Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  5. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.
  6. Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care.
  7. Supports the Case Managers and assumes responsibility to coordinate patient care for the agency after hours.
  8. Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
  9. Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
  10. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
  11. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
  12. Actively participates in quality assessment performance improvement teams and activities if needed.
  13. Perform additional job duties as assigned.
Job Qualifications:
  1. Bachelor's or Associate's degree in nursing or related fields, required.
  2. Maintains an active nursing license that is in good standing in the state performing job duties.
  3. Maintains an active CPR certification.
  4. At least 2-5 years of experience in the healthcare management, hospice, or palliative care and/or training in end-of-life care, preferred.
  5. At least 1 year of experience with Medicare Hospice Certification as well as CHAP guidelines, preferred.
  6. Knowledge of hospice care and the services provided to patient and family/caregiver through an interdisciplinary group.
  7. Proficient in MS Office applications and ability to learn department and job-specific software systems.
  8. Demonstrate organizational skills.
  9. Demonstrate effective verbal and written communication skills.
  10. Demonstrate analytical skills when problem-solving.
  11. Demonstrate high attention to detail and a high degree of accuracy.
