Job summary

Hope Network in Michigan is seeking an On-Call Physical Therapist to support the Neuro Rehabilitation program. You will evaluate, treat, and educate individuals recovering from neurological injuries while collaborating with an interdisciplinary team to improve mobility and quality of life.

In this flexible, on-call role, you will contribute to care planning and discharge planning, with consistent scheduling opportunities anticipated during maternity leave coverage.

Qualifications

• Clinical reasoning and problem solving to guide therapy decisions.
• Clear communication and collaboration with multidisciplinary team.
• Strong documentation and organizational skills for timely notes.

Responsibilities

• Evaluate individuals and develop individualized PT plans.
• Provide hands-on therapy to improve strength, balance, mobility, and independence.
• Design and monitor home exercise programs and mobility interventions.
• Train direct care staff on safe transfers and gait techniques.
• Collaborate with PTAs, rehab staff, physicians and team members.
• Complete timely documentation, progress notes, and reports.
• Participate in care and discharge planning, family education, and meetings.
• Monitor equipment and recommend adaptive devices as needed.
