Keizer Nursing & Rehabilitation Center in Keizer, OR is seeking a Licensed Practical Nurse (LPN) to join our team. The role offers a new wage scale starting from $44.73/HR to $60.34/HR, based on licensed experience, with PRN shifts and non-guaranteed scheduling.

As part of Volare Health family, you will lead and mentor CNAs, assess resident needs, and help develop personalized care plans while ensuring regulatory compliance and compassionate care.

Qualifications

• Must hold a valid and active LPN license in Oregon (Required).
• Ability to assess and address resident needs while developing personalized care plans.
• Provide guidance and supervision to CNAs and other care staff.

What We’re Looking For:

LPN License:Must hold a valid and active LPN license in Oregon.(Required)

• Availability to work 1-2 shifts per week, 2 weekend shifts per month (Required)

Resident Care:Ability to assess and address resident needs while developing personalized care plans.

Team Leadership:Provide guidance and supervision to CNAs and other care staff, ensuring the highest level of care delivery.

Passion for Care:A genuine desire to improve the lives of others and uphold the highest standards of nursing care.

Regulatory Knowledge:Familiarity with state and federal regulations governing nursing care.

New nurses can participate in a 3-month Long Term Care New Nurse Residency, involving the Geri-Res residency curriculum and a structured onsite mentorship.

Additional program benefits include:

• Professional development opportunities through OHCA, available at no cost to Nurse Residents.
• For a limited time, Nurse Residents are eligible to apply for a Long-Term Care New Nurse Incentive payment following completion of the residency and three additional months of employment. (The grant that funds this incentive expires at the end of 2026; nurses must be enrolled by June 30, 2026 to be eligible.)
• The residency program bridges the gap between academic learning and real-world practice, helping new nurses transition smoothly into long term care. Experienced nurses new to long term care are also eligible.
