This Mobile Health Specialist role centers on on-call, patient-centered care with a focus on flexible work in Long Island. It emphasizes home visits, collaboration with telehealth Emergency Physicians and care coordinators, and a proactive, on-demand care model.

Qualifications

• Valid NYS Paramedics Certification.
• 3 years of experience in 911, Interfacility or Community Paramedicine.
• BCLS, ACLS certifications and NYS Paramedic license; valid NYS driver's license.

Responsibilities

• Home Visits: Conduct in-person home visits to manage acute illnesses and injuries.
• Treatment & Collaboration: Provide treatment per evidence-based guidelines and collaborate with a multi-disciplinary team including telehealth Emergency Physicians and care coordinators.
• Risk Assessment: Identify risk factors such as medication compliance, limited mobility, impaired nutrition, fall safety, and social isolation.
• Documentation & Compliance: Maintain patient confidentiality, complete timely electronic records, and adhere to acute care standards.
• Vehicle Requirements: Respond on an on-call basis using a personal vehicle, with option to use a Care2U vehicle when available and pick up equipment from designated locations.
