PruittHealth Hospice is seeking a Registered Nurse Case Manager to join our on-call team for weeknights (7 on, 7 off) in Georgia. You will evaluate, plan, and coordinate hospice patient care in home or facility settings.

Qualifications

• Complete all documentation and records in a timely and accurate manner
• Follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a hospice setting
