Job summary

Mt Carmel Hospice in Columbus, OH is seeking an On-Call Registered Nurse to provide skilled end-of-life care. This full-time role covers 5:00 PM to 8:00 AM on a 7-on/7-off rotation, addressing after-hours patient needs and coordinating with the hospice team.

Applicants must hold an Ohio RN license and CPR certification; hospice, home health, oncology, critical care, ER, or med-surg experience is preferred. Travel throughout the greater Columbus market is required to deliver compassionate,

Qualifications

Active Ohio RN license.
Current CPR certification.
Hospice, home health, oncology, critical care, ER, or med-surg nursing experience preferred.
Ability to travel throughout the greater Columbus market.
On-call rotations are required.

Responsibilities

Perform comprehensive patient assessments and evaluate changes in condition.
Provide symptom management and end-of-life nursing care.
Respond to after-hours, weekend, and holiday patient needs.
Coordinate care with physicians, hospice leadership, facilities, and interdisciplinary team members.
Complete admissions, routine visits, crisis visits, GIP, and CC visits as assigned.
Educate patients and caregivers regarding disease progression, medications, and comfort measures.
Ensure accurate and timely EMR documentation and compliance with hospice regulations.
Participate in quality improvement initiatives, team meetings, and ongoing professional development.
