Job summary

Halcyon Home LLC is seeking a dedicated Hospice LVN On-Call to provide triage and clinical support to hospice patients and families. The role follows a 7 days on, 7 days off schedule with competitive pay and strong team support.

Ideal candidates are Texas-licensed LVNs with hospice or acute care triage experience, excellent communication, empathy, and the ability to work independently under pressure. This position supports home health and home care teams as needed.

Qualifications

• Active LVN license in the state of Texas or multi-state license.
• Minimum of 2 years in hospice, palliative care, or acute care; triage experience preferred.
• HomeCare HomeBase experience is a plus.
• Excellent communication, problem-solving and critical-thinking skills.

Responsibilities

• Conduct thorough triage assessments via telephone.
• Provide symptom management guidance and education to patients and families.
• Coordinate with on-call hospice team to facilitate timely care.
• Maintain accurate documentation in the patient care system.
• Offer emotional support to patients and families throughout the care process.
• Assess urgency of care and escalate cases when needed.
• Communicate with interdisciplinary teams to update care plans.
