Qualifications

• Graduate of an accredited LVN/LPN program and current NM license.
• CPR certification required and a driver’s license with auto insurance.
• 1 year of nursing experience; Hospice experience preferred.

Responsibilities

• Administer medications and treatments per hospice policies.
• Perform baseline nursing skills with clinical competency.
• Monitor patient condition and report changes to RN.
• Develop and update Plans of Care with measurable goals.
• Document nursing interventions and education provided.
• Provide emotional support to patients and families.
• Communicate with physicians and interdisciplinary team.
• Participate in team conferences and care planning.
• Support fellow staff to promote teamwork.
