HealthCare Connections, Inc. in Oklahoma City is seeking a Nurse Practitioner to deliver post-acute care across multiple locations. You will operate on a Monday–Friday schedule, rounding and going to patients, in an employed role with a salary plus wRVU bonus.

A 401k with employer match is offered to support long-term planning. The ideal candidate holds an active Oklahoma NP license and has post-acute care experience.

Qualifications

• Active Oklahoma NP license required.
• Experience in post-acute care preferred.

• Provide post-acute care services across multiple OKC locations.
• Maintain a Monday–Friday schedule for rounding and go visits.
