Job summary

Evolve Therapy Services in Minneapolis, MN seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. This PRN role supports planning, directing, and delivering occupational therapy services to residents with supervision from an OT.

Applicants must hold an active OTA license in applicable states, complete continuing education, and collaborate with the interdisciplinary team to meet resident goals and ensure compliant documentation.

Qualifications

• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Associate's degree in Occupational Therapy completed.

Responsibilities

• Plan, organize, develop and direct Occupational Therapy services in the facility.
• Screen residents and determine best intervention based on scope of practice.
• Implement treatment plans and monitor progress toward goals.
• Communicate evaluation findings and recommendations to residents and team.
• Establish resident therapy schedules in coordination with team members.
• Document and bill OT services per treatment plan and CMS standards.
• Perform other duties as assigned to ensure quality resident care.

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
