Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Clarion, PA. This PRN role supports resident care and requires passion, professionalism, and licensure in the applicable state.

You will screen residents, implement treatment plans, and work with an interdisciplinary team to maintain CMS-compliant documentation and billing practices.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to identify best intervention based on scope of practice.
• Execute treatment plans and monitor progress toward goals per plan of care.
• Communicate evaluation findings and recommendations to residents, families, and team members.
• Coordinate resident therapy schedules with team members.
• Deliver therapy according to plan and ensure proper documentation and billing of occupational therapy services.
• Perform other duties as assigned and ensure CMS documentation standards are met.

Job Position: Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patientsComply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
