Evolve Therapy Services in Gatesville, NC is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. This role supports therapy services and collaborates with a multidisciplinary team to promote resident independence.

Responsibilities include conducting screenings, implementing treatment plans, documenting progress, and ensuring CMS compliance. Must hold OTA licensure and an associate degree in Occupational Therapy, with ongoing continuing education.

Qualifications

• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment.
• Perform treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families/caregivers, and team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to the treatment plan and document/bill OT services.
• Comply with CMS documentation standards daily.
• Perform other duties as assigned to effectively treat patients.

Job Position: Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
