Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant to provide in-house therapy at a Skilled Nursing Facility in Trenton, NJ. The role requires an OTA license and an Associate's degree, with standard full-time responsibilities in patient assessment, treatment planning, and documentation.

You will collaborate with a multidisciplinary team to ensure high-quality resident care, maintain CMS compliance, and participate in ongoing professional development for licensure

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Continued education and licensure maintenance required.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Perform treatment plans and follow short- and long-term goals established by supervising therapist.
• Interpret and communicate evaluation findings to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules with team members.
• Conduct therapy according to treatment plan and document and bill occupational therapy services.
• Perform other duties to effectively treat patients.
• Comply with CMS documentation standards and routine point-of-care documentation.

Education

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
