Job summary

Evolve Therapy Services, LLC is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Byron, MN. This full-time position focuses on delivering occupational therapy services within a team-based setting and ensuring high-quality resident care.

The OTA will screen residents, implement treatment plans, and maintain thorough documentation while coordinating with supervising therapists and CMS requirements.

Qualifications

• Associate's degree in Occupational Therapy is required.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine best intervention based on scope of practice.
• Perform treatment plans and follow short- and long-term goals.
• Communicate evaluation findings and recommendations to the care team.
• Establish individual resident therapy schedules with the team.
• Document and bill occupational therapy services per plan.
• Perform other duties as needed to treat patients.
• Comply with CMS documentation standards daily.

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

Full Time

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
