Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Madison, WI. This PRN role supports the therapy team and focuses on delivering high-quality resident care in coordination with supervising therapists.

The role requires an Associate's degree in Occupational Therapy, active OTA licensure, and commitment to ongoing professional development.

Qualifications

• Associate's degree in Occupational Therapy is required.
• Must hold active OTA licensure in the state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Implement treatment plans aligned with goals and plan of care.
• Explain evaluation findings and plans to residents and families.
• Coordinate therapy schedules with team members and manage documentation.
• Deliver therapy according to the plan and ensure proper billing and records.
• Perform additional duties to support patient care as needed.
• Follow CMS documentation standards in daily practice.

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice


• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist


• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members


• Establish individual resident therapy schedules in cooperation with other team members


• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services


• Perform all other duties as assigned and as required to effectively treat patients


• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy


• MUST have active OTA licensure in state(s) of practice


• Ongoing continuing education and professional development to maintain current licensure and certification
