Evolve Therapy Services, LLC in Johnstown, PA is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. This full-time role focuses on planning, organizing, and directing Occupational Therapy services to ensure high-quality resident care in accordance with federal, state, and local standards.

You will screen residents, implement treatment plans, communicate findings to families and the clinical team, and manage therapy schedules while documenting and billing

Formación

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsabilidades

• Conduct regular screening of residents to determine best intervention based on scope of practice.
• Perform treatment plans and follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings and recommendations to residents, families, and interdisciplinary team.
• Establish individual resident therapy schedules in collaboration with team members.
• Deliver therapy according to the established plan and document and bill occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily with point-of-care documentation.

Educación

Associate's degree in Occupational Therapy

MUST have active OTA licensure in state(s) of practice

Ongoing continuing education and professional development to maintain current licensure and certification

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
