Job summary

The Lakes Home Care, Inc. is seeking an Occupational Therapist Assistant to join our team in New York. You will work in outpatient home health settings to help clients improve daily living and work skills, under supervision and within coordinated care plans.

Responsibilities include assessments, developing treatment plans, providing therapeutic exercises, and educating patients and families on adaptive techniques and equipment. Flexible scheduling and team-focused care are offered.

Qualifications

• Experience in outpatient settings and OT terminology is required.
• Strong knowledge of daily living activities (ADLs) and patient care.
• Familiarity with home health care delivery across settings.
• Experience in occupational health and clinic-based care is a plus.

Responsibilities

• Conduct patient assessments to determine individual needs and goals.
• Develop personalized treatment plans to improve patients' daily activities.
• Provide therapeutic exercises to enhance motor skills and coordination.
