Job summary

Therapy Management Corporation (TMC) in Toledo, Ohio, seeks a Certified Occupational Therapy Assistant (COTA) to deliver patient-centered therapy to adult and geriatric patients. You will help improve independence, mobility, safety, and quality of life in a collaborative clinical setting.

You will follow patient progress under an OT, develop functional treatment plans for ADLs and mobility, document outcomes, and contribute to quality care with clinical autonomy and ongoing professional growth.

Qualifications

• Must be a COTA or license eligible in the state of practice.
• Graduate of an accredited Occupational Therapy Assistant program.
• Interest or experience in geriatric rehabilitation or senior living therapy.
• Commitment to patient-centered care and improving quality of life for older adults.

Responsibilities

• Provide personalized occupational therapy services under the direction of an OT to improve functional independence.
• Implement patient-centered treatment plans focused on ADLs, mobility, and safety.
• Help patients perform daily tasks such as dressing, bathing, grooming, and meal prep.
• Collaborate with interdisciplinary teams to support outcomes and continuity of care.
• Maintain accurate clinical documentation and monitor patient progress.

What You’ll Do:

• Provide personalized occupational therapy services under the direction of an Occupational Therapist to help patients improve functional independence, safety and overall quality of life
• Implement patient-centered treatment plans focused on ADLs, strength, mobility, and adaptive techniques
• Help patients improve their ability to perform daily tasks such as dressing, bathing, grooming, and meal preparation
• Partner with interdisciplinary teams, caregivers, and nursing staff to support strong patient outcomes and continuity of care
• Maintain accurate clinical documentation, monitor patient progress and adjust treatment plans based on individual rehabilitation needs
