Job summary

Blue Stone Therapy in Iowa is seeking a Credentialed Occupational Therapy Assistant to join our rehabilitation team serving adult and geriatric patients in skilled nursing and post-acute settings.

Under supervision of a licensed OT, you will implement plans of care, document progress, and collaborate with an interdisciplinary team to promote independence and ADLs.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current Occupational Therapy Assistant license (or eligibility for licensure) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong communication, organization, and clinical reasoning skills.
• Commitment to patient-centered care and professional standards.

Responsibilities

• Provide skilled occupational therapy treatment in accordance with established plans of care.
• Implement individualized treatment programs focused on functional and measurable outcomes.
• Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings.
• Monitor patient response to treatment and communicate progress with the supervising Occupational Therapist.
• Maintain timely, accurate documentation to support quality care and reimbursement standards.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.

Occupational Therapy Assistant (COTA)

Blue Stone Therapy is seeking a TEMPORARY FULL-TIME, PART-TIME, OR PRN Occupational Therapy Assistant (COTA) to provide rehabilitation services to adult and geriatric patients in Shell Rock and Clarksville, Iowa.

This role focuses on delivering skilled occupational therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Occupational Therapy Assistant works under the supervision of a licensed Occupational Therapist to implement treatment plans designed to improve activities of daily living (ADLs), functional mobility, strength, cognition, and overall independence.
