Job summary

Concordia Home Health and Hospice of Bethlen is seeking a full-time or part-time Occupational Therapist (OTR/L) to cover Ligonier, Latrobe, Greensburg and surrounding areas. The OT directs, evaluates and provides therapy services in the patient’s home as prescribed by the physician.

The position emphasizes initial evaluations, treatment planning, and collaboration with the nursing team to support independence and safe home environments.

Qualifications

• Must be registered and licensed Occupational Therapist.
• Licensed in Pennsylvania.
• Home health experience is preferred.

Responsibilities

• Make home visits to patients in the Ligonier Valley and surrounding service area.
• Perform initial OT evaluation and assess functional status, endurance, coordination, motor skills, and barriers to independence.
• Set up and implement the treatment plan and coordinate with nursing for multidisciplinary care.
• Report and document care provided for home health and hospice patients under physician direction.
• Identify equipment needs and environmental adaptations.
• Educate patient, caregivers and family in rehabilitative care.
• Supervise the Certified Occupational Therapist Assistant in implementing the plan of care.
• Revise therapy plans as needed to improve patient outcomes.

The licensed Occupational Therapist OT responsibilities include:
