Proforma Health is seeking a dedicated Occupational Therapist to help patients regain daily living skills after injuries or illnesses. The role involves delivering therapy across multiple settings with potential float assignments to different clinics.

Qualifications

• Must hold an active State License to practice Occupational Therapy.
• Bachelor’s, Master’s, or Doctoral degree in Occupational Therapy from an accredited program.
• BLS certification may be required (AHA or ARC).
• Strong organizational and communication skills.

Responsibilities

• Evaluate and develop personalized treatment plans for patients.
• Assist patients in achieving functional goals to enhance independence.
• Manage a patient load of approximately 12-14 patients daily, with expectations of 60 visits per week.
• Handle follow-up visits, including double-booking as needed.
• Float to multiple clinics as required to meet patient needs.
