Job summary

Plan A Health is seeking an Occupational Therapist to provide therapy in a skilled nursing facility setting, helping patients regain daily skills and independence.

You will assess patients, develop treatment plans, and collaborate with care teams to ensure high-quality care while supporting recovery and functional progress in a stable, flexible environment.

Qualifications

• OT degree from an accredited program (Bachelor, Master, or Doctoral).
• Active State License required or in process.
• BLS Certification may be required.
• Experience in skilled nursing facilities preferred; new graduates welcome.

• Provide therapy services to a caseload of 16-20 skilled patients.
• Assess and develop individualized treatment plans.
